You may use our patient portal to review your medical history, medications, and allergies. It may also be used to access some of the medical information associated with your visits such as lab results and diagnoses made.
Your portal account will be available after your first visit to the office. If you are an existing patient, but were last seen prior to September 2013 (when our electronic health record was implemented), your portal account will be available after your next office visit.

The patient portal link connects you directly to the portal where you will need the username and password provided by our receptionist at the time of your appointment.
If you did not receive or have misplaced your username and password, please contact us at 319-339-3872 and we can provide you with your username and a new password.
